To ensure your child’s health needs are safely supported during the school day, the following forms may be required:
Medication Forms

Over-the-Counter Medication Form
Required for medications like cough drops, Tylenol, or Ibuprofen. Herbal remedies are not FDA-approved and follow specific Alaska protocols. Please consult our school nurse if you plan to send any non-prescription medications. -
Short-Term Medication Form
For prescribed medications (like antibiotics) needed at school for 2 weeks or less. -
Prescription Medication Form
Needed for students who require ongoing prescribed medication during the school year.
Care Plans

Anaphylaxis Care Plan
Required for students with life-threatening allergies and a prescribed EpiPen—even if your child self-carrys. -
Asthma Care Plan
Needed for students with asthma who may require an inhaler at school, even for those who self-carry. -
Seizure Care Plan
For students diagnosed with epilepsy who may need medication or emergency support at school.
Other Important Forms

Medical Statement for Special Meals
For students needing cafeteria meal substitutions due to allergies, lactose intolerance, or other medical conditions. Must be signed by a healthcare provider and is valid until updated. -
Religious Exemption to Immunizations
Must be completed yearly after July 1st and notarized.
