🩺 Health Forms & Care Plans at Alpenglow

  • To ensure your child’s health needs are safely supported during the school day, the following forms may be required:

    Medication Forms

    • Over-the-Counter Medication Form
      Required for medications like cough drops, Tylenol, or Ibuprofen. Herbal remedies are not FDA-approved and follow specific Alaska protocols. Please consult our school nurse if you plan to send any non-prescription medications.

    • Short-Term Medication Form
      For prescribed medications (like antibiotics) needed at school for 2 weeks or less.

    • Prescription Medication Form
      Needed for students who require ongoing prescribed medication during the school year.

     


     

    Care Plans

    • Anaphylaxis Care Plan
      Required for students with life-threatening allergies and a prescribed EpiPen—even if your child self-carrys.

    • Asthma Care Plan
      Needed for students with asthma who may require an inhaler at school, even for those who self-carry.

    • Seizure Care Plan
      For students diagnosed with epilepsy who may need medication or emergency support at school.
